Type 2 Diabetes Self Assessment

Diabetic Clinics

Diabetic clinics are held twice weekly at Lakeside Health Centre and aim to provide a high standard of care, education and support to all our diabetic patients and their families.

They are seen by Jackie Hall –Lead Diabetic Practice Nurse with specialist training and qualification in both Type 1 and Type 2 diabetes. She is supported by Dr Maryam (who oversees more complicated patients and runs specialist clinics twice monthly).

Annual reviews, diet and lifestyle education including referral to X-Pert education and regular monitoring is performed. Those patients who require insulin initiation together with dose adjustment are also seen and supported within the practice thus saving needless journeys to Secondary Care.

By supporting our patients and monitoring them regularly we aim to recognise and reduce the risks of long term diabetic complications and to enable our patients to lead a healthier lifestyle.
